Perimenopause

Ask most people what perimenopause looks like and they will say hot flashes. But the largest recent survey of women living through it tells a very different, and far less talked about, story.

In its 2026 State of Menopause survey, Bonafide asked more than 2,000 women aged 40 to 64 about their experience. The headline finding is one almost no one prepared them for: the hardest part is not physical.

It Was Never Just About Hot Flashes

In the survey, 60 percent of women said their symptoms affect their emotional or mental health, and 77 percent reported cognitive changes like brain fog. When asked which symptom hit hardest, anxiety and low mood ranked number one, ahead of hot flashes.

For a life stage that half the population will move through, that is a striking gap between the story we are told and the reality women describe.

The Loneliest Part Is Feeling Unseen

The emotional weight showed up again and again. In the survey, 71 percent of women did not even recognize their first perimenopause symptom as perimenopause. Around 69 percent said they wanted to be alone more often, and 74 percent found themselves annoyed by things that never used to bother them.

Most telling of all: 85 percent said the best way to navigate this stage is to educate themselves, because too often no one else did. Read that again. The most common survival strategy for a universal life stage is figure it out yourself.

“You Are Not Imagining This”

The survey found something quietly moving. When women talk to each other about perimenopause, they are not first looking for a product or a fix. Sixty-two percent said the single biggest benefit of the conversation was simply realizing they were not imagining it.

Before solutions, before supplements, before anything, women need to hear one thing: this is real, and you are not alone in it.

Why Perimenopause Gets Overlooked

This connects to a much bigger story. Women were only required to be included in federally funded clinical research in 1993, and midlife women’s health has been among the most under-studied areas of all. It is not that women were imagining their experience. It is that, for decades, almost no one was studying it.

The good news is that this is finally shifting, both in national policy and in the wider culture. But the women living it right now still deserve to feel seen today, not in ten years.

What Support Can Actually Look Like

Perimenopause is not a switch that flips. For many women it is years of a cycle that slowly changes its pattern. The most useful thing is not a rigid rulebook handed down by a stranger. It is paying attention to your own body as it shifts, and meeting it with steadiness and compassion instead of white-knuckling through changes no one explained.

Food first. Rest when your body asks. Notice your own pattern. And offer yourself the same kindness you would give a friend who told you she felt like she was on the outside looking in.
